Insurance
Dr. Maier will be working with many health insurance companies, including:
- Aetna
- Anthem Blue Cross Blue Shield
- Cigna
Health insurance. Health insurance covers a 15 minute appointment with Dr. Maier. If you prefer a 30 minute appointment, we offer a discounted self-pay rate for the additional time and treatment. We will bill your insurance on your behalf. You will be required to provide a valid insurance card and any applicable co-payment, co-insurance, or deductible at time of service.
Auto Insurance. Auto insurance covers a 30 minute appointment with Dr. Maier. We will bill your insurance on your behalf; you will be required to provide valid claim information and any required documentation.
Self-pay Fees
If we will not be billing insurance for your treatment, we offer the fees below that reflect a prompt-pay discount.

Cancellation Policy
We require 12 hours notice to change or cancel your appointment. We reserve the right to charge a $50 fee if you reschedule/cancel without providing 12 hours notice (absent a compelling reason) or fail to keep your appointment (“no show”). This fee is not covered by insurance and must be paid prior to your next appointment. Thank you for your understanding as we strive to serve the needs of all our patients!
Financial Responsibility
Each patient is financially responsible, whether or not their insurance company pays, for all charges they incur. Medical insurance benefits, including Medicare, private insurance, and other health plans are assigned to the practitioner (i.e., the insurance company payment will go directly to the provider). Each patient authorizes the release of any protected health information required to secure payment.
Late Payment
Any account over 60 days due will be sent to collection. If you default payment, you will be responsible for all costs of collections including but not limited to collection agency fees, attorney fees and court costs. Payment arrangements should be made prior to the account being referred to collections.
